Before your first visit

Bring a photo ID, your insurance card if you have one, and a list of anything you take regularly: prescriptions, supplements, blood thinners. If another office took X-rays of your teeth in the past year, ask them to send the files over. We can often read those instead of repeating them.

If a tooth is hurting right now, call instead of writing. The phone reaches someone during office hours; a form sits until somebody opens it.
